For specimens that you physically send to IdentifyUS, please print and send a copy of your Specimen Evaluation Form along with your sample. When you fill out this form on-line, an electronic copy will be emailed back to you. Print it out and include it with your specimen when and if you send it to us for further examination or testing. Don't forget to include and attach digital images of your specimen (if you can) when you fill out and complete the form (below). Refer to our Tips, Tricks & Techniques page for suggestions on how to capture the best possible pictures of your sample and how to prepare specimens for submission.
